From the CardDefinitionForm Actions button bar:
▪Click on the Create new option
this open up the Create new Card Definition popup window
▪Fill in the mandatory Name field and any other required ones, including Vendor.
The Height and Width fields should be in relation to the Equipment Definition's slot definition
▪Click on the Save icon in the lower left corner of the popup window
