From the CardCodeDefinitionForm Actions button bar:
▪Click on the Create new option
this open up the Create new CardCodeDefinition popup window
▪Fill in the mandatory CardDefinition and Name fields and any other required ones
The Height and Width fields should be in relation to the Equipment Definition's slot definition
▪Click on the Save icon in the lower left corner of the popup window
